[IGSTK-Developers] process book chapter

Patrick, Kevin,

I've got what I think is a chapter ready for a first review by the
editor. I added a screenshot from DART that doesn't look that
great though. It was created from a jpeg - can you suggest a
different way, or does anyone have a nicer looking shot handy?

I also added two appendix files and references in the top-level
IGSTKTheBook.text file and checked those in as well. One is the
IGSTK style guide - all I have for this is a January 2005 PDF.
Do we have something later? And who has an original (it would be
great if there was already a Latex version!)?

I added a placeholder for a short 2nd appendix on state machine
validation. I will added that section soon.
