used under the provisions of the IGSTK copyright
found at http://www.igstk.org/copyright.htm"
First of all:
=============
The statement confuses a "copyright" with a "license". In the
last sentence, it probably should say "under the provisions
of the IGSTK *license* found at...".
A copyright doesn't have provisions, the license is the
one that establishes the allowed uses of the copyrighted
material.
Second:
=======
The first statement is contradictory with the last one.
Namely, the first statement says that no part of the book
can be reproduced by any means, but the license in
"http://www.igstk.org/copyright.htm" states that:

"Redistribution and use in source and binary forms, with or without
modification, are permitted provided that the following conditions
are met:
* Redistributions of source code must retain the above copyright notice,
this list of conditions and the following
* Redistributions in binary form must reproduce the above copyright
notice, this list of conditions and the following disclaimer in
the documentation and/or other materials provided with the
distribution.
* Neither the name of the Insight Software Consortium nor the names
of its contributors may be used to endorse or promote products
derived from this software without specific prior written permission.

Which indicates that redistribution is allowed.
Third:
======
Inserting a restrictive note on a document whose electronic version
is posted on the Web under a BSD license is a great contradiction.
(not to mention that is ineffective)
In any case, is there any reason the reproduction and redistribution
of the book should be restricted ?
As far as I remember, the book is not being commercialized. Therefore,
instead of restricting the redistribution and reproduction of the book
we should rather encourage it, in particular, as a way of promoting the
use of the toolkit and as a mechanism for growing the community.
Unless anybody has an objection, I propose that we remove this hostile
and incongruous paragraph from the book.
