[Insight-developers] Hashtable errors and kwsys

We need to resolve the lingering hast_table errors on Visual Studio. Perhaps
we should backtrack and use the itk_hash_x stuff since the kwsys stuff is
not currently portable and apparently untested.

We can't afford to have these nightly compiler errors:
