in particular the chapter on

     "Reading and Writing Images"

where the DICOM writing is described.

You probably missed the line 339 of

   Insight/Examples/IO/
       DicomSeriesReadSeriesWrite.cxx

with the call to the method

       SetMetaDataDictionary()
