[Insight-users] how to write mhd ?

hi,everyone,
   I have a RAW data of MR image,and its format is 256*256*256, with 16bits stored and 12bits effective. I want to write a mhd/mha file for it .I tried to write it ,but it was wrong.
               NDims = 3
               ElementByteOrderMSB = False
               ElementSpacing = 0.36 0.36 0.5
               DimSize = 256 256 256
               ElementType = MET_UCHAR
               Offset = 0 0 0
               ElementDataFile = patient.raw
could you tell me to I write?
