[Insight-users] continuous voxel indices

There are 3 coordinates:
Index (integral voxel coordinates e.g. 100, 243
ContinuousIndex (continuous voxel coordinates e.g. 100.3, 243.7)
Physical (using origin, direction and spacing converts from index or
continuous index to physical (e.g. mm) coordinates).
