[Insight-users] Append two 3d images to one

I would suggest to look at the following, in order of likely hood that I think they'll do what you want:

TileImageFilter, JoinSeriesImageFilter, and PasteImageFIlter.

Use caution when setting the LayoutArray with the TileImageFilter.
