Hi Vincent,

Thanks a lot for your rapid response.

Here is the form that must be filled in order to transfer
the Copyright of source code to the Consortium:

http://www.insightsoftwareconsortium.org/documents/policies/OSCertification-v2006-02-01.pdf

The form must be faxed to Josh Cates at the fax number
appearing at the top of the text.
