DCMTK will read DICOMDIR files. Basically, a DICOMDIR is just an index of a
bunch of DICOM files to save software the hassle of opening each file in
turn. Since cracking a DICOM header is very inexpensive with the right
software, DICOMDIR files are not that interesting any more.
Your best bet is to use GDCMSeriesFileNames on each _series_ directory.
Won't help much at the study level.
There are few, if any tools to convert DICOM to mha. You may need to use
ITK for that.
