Hi,
I'd suggest plastimatch's reg-2-3 (www.plastimatch.org). It's an ITK/VTK/Qt-based 2D/3D intensity-based n-way registration framework meaning that its is capable of registering 1,2..n 2D projective images to one 3D volumetric image by a forward projection approach (DRR generation). Thus, it should be suitable for your purpose.
